Statement of Assets & Liabilities
Evidence of services rendered, such as:
          -Properly accomplished DTR
Certification of assumption to duty

2. SALARY DIFF. UNDER PROMOTIONAL APPT.

Certified true copy of appointment
Properly accomplished DTR
Certificate of assumption in new office

3. SALARY DIFF.INCASE OF GEN.SALARY INCREASE/ACROSS THE BOARD

Salary adjustment notice (NOSA)
Monthly Report of Attendance (MRA)/DTR

4. SALARY UNDER APPOINTMENT BY TRANSFER

Copy of authority to transfer
Clearance from money and property accountability from former office
Certification of last day of service in former office
Certificate of last salary received certified by the Chief Accountant
Certified copy of appointment
Certification of leave credits
Certification of assumption to new office
DTR
Statement of Assets & Liabilities
Oath of office

5. VACATION/SICK LEAVE

Approved application for leave
Medical certificate for sick leave exceeding 5 days
Clearance from money and property accountability, if leave is 30 days or more

6. MATERNITY LEAVE

Approved maternity leave of absence
Clearance form money and property accountability
Medical certificate of attending physician

7. TERMINAL LEAVE

Approved terminal leave
Complete service record
Clearance from money and property accountability
Statement of Asset & Liabilities
Copy of last appointment and appointment showing highest salary received if the
          salary under last appointment is not the highest.
Certification that no pending administrative case (City Legal Office)
